GP Attachment in Newmarket Day 9

Assalamualaikum..


Hari ni mungkin lah taknak cerita secara klinikal sangat, tapi more of personal feeling. So, hari ni first first pagi-pagi pukul 9.15am dah sampai dekat rumah GP. Partner tinggalkan kereta kat situ, so then kitorang pergi klinik desa kat Lisscaroll dengan kereta GP. 


Pastu masa kat klinik desa, sekali GP kata suruh dinner rumah dia pukul 6.30pm. Aku macam, haaa dinner lol, nampaknya lambat lah balik B&B hari ni kalau dinner kat rumah GP. Then dah pukul 10.30am, gerak pergi klinik kat Newmarket. Lama gila kot duk kat klinik. Pergi lunch kejap. Pastu belah petang, follow pergi Kanturk Community Hospital, lepas tu balik klinik kat Newmarket dalam pukul 3.30pm. And then duk kat klinik sampai pukul 6.00pm. HAHAHA


Ya Allah, penat gilaaa~ Rasa macam energy dah habis licin bersosial di klinik. Pastu balik rumah GP balik sebab kereta partner kan kat situ. So yeahh, dinner kat rumah GP. Suasana dah la malap, maklumlah lampu pun lampu oren. Mula-mula lepak kat ruang tamu dulu and then GP suruh anak perempuan dia hidangkan soft drink untuk aku. Pastu GP tanya kat partner nak wine apa...


Lol~ Rasa macam awkward moment tahap apa je. First time in my life kot duduk semeja, makan malam, atas meja ada wine. Ya Rabbi. Tadi makan complete set pulak. Appetizer - sup brokoli potato. Main dish - ikan masak apa entah dengan sayur dia steamkan kot dengan potato. Aku tak ambik potato, pastu GP cakap, that's our rice, HAHAHA. Kiranya aku tadi makan lauk je la. Catat rekod dua hari tak makan nasik, mohon weight loss~ And then dengan dessert aiskrim dengan apple pie. Kenyang sangat Alhamdulillah..


Pastu kat meja makan tu ada lah anak perempuan GP 16 tahun, anak lelaki dia jugak umur dalam 26 tahun macam tu. Banyak gila sembang, exhausted dah nak follow. Pastu jam dah pukul 9.30pm, baru lah undur diri.


Okey lah, let's get back to clinical stuffs.


Hari ni ada 20 years old boy datang GP sebab dia ada cough. So kena check temperature dia. First time dapat try guna alat kat bawah ni. So yeah, he got fever. And then bila auscultate lung dia, macam dull sikit bahagian kiri and ada jugak difference vocal fremitus belah kiri dan kanan. So GP expect dia ni ada chest infection. So kalau chest infection kita bagi apa? Antibiotic... So patient ni before pun pernah ke chest infection and antibitotik yang work on him is Amoxycillin. So GP pun bagi the same antibiotic.




Budak laki ni jugak dia merokok sebenarnya.. So GP pun consult kat dia, berapa banyak money he can save kalau dia stop merokok. Sebab bila merokok pun make somebody susceptible laa getting infection.


Next masa pergi community hospital, ada seorang makcik ni ada cellulitis kat kaki kiri dia. Memang merah habis laa pastu macam ada ulcer. Tapi GP kata dia bukan diabetic pun. Suggestion GP, makcik tu kena admit dalam the real hospital laa untuk further management in term of nak masuk IV fluid ke apa. Tapi anak makcik tu macam reluctant, so GP prescribe high dose flucloxacillin. 


And then lagi satu pasal South Doc. Banyak kali dengar diorang cerita pasal South Doc, tapi aku takde idea langsung. 


Established in 2001, the SouthDoc co-op was set up to serve the people of Cork and Kerry, and visitors to the area. It is part of the primary care network within the Health Service Executive (HSE). The co-op has a membership of almost 500 Doctors, including your own GP. Your consultation will usually be with a local GP, or indeed your own GP.

What happens when I call SouthDoc?
Your call will be answered by a member of staff from the SouthDoc base and you will be asked to give details of the patient and a contact phone number. If you have a medical card you will be asked for the number and expiry date.
A Triage Nurse will ring you back as soon as possible thereafter. The Nurse will gather all the relevant medical information and make an assessment which will lead to one of the following:

If the details given suggest a minor medical problem, the Nurse will give advice to the patient/guardian.
Alternatively, a Doctor may ring back to discuss the issue and give advice.
If you need to see a Doctor, the next available appointment will be offered for the patient to attend a doctor at the SouthDoc centre nearest to you.
For patients who are seriously ill or suffer from lack of mobility, a house call may be offered.
If the situation is very urgent, an ambulance may be called on your behalf.
After your consultation, details will be sent to your own GP by the following morning.
