GP Attachment in Newmarket Day 2

Assalamualaikum.


Another day in GP attachment. So hari ni sebelah pagi spend time dengan nurse. Ada seorang pakcik tu cakap kat nurse tu, "your student is so happy looking,". HAHA. Ye lahh, takkan aku nak buat muka serius pulak haha. Oh lupa nak tulis semalam, ada seorang pakcik tu teka yang aku from Iran. Pastu aku kata dia, nope, would you make another guess? Dia jawab Iraq. Ya, aku rasa muka aku memang takde genetik muka Indon, Thailand atau South East Asian kot. 


Basically saja nak jot down few things yang aku belajar masa dengan nurse tadi. Antaranya EPO injection. First of all, apakah itu EPO? 


Erythropoietin (EPO) is a hormone that prevents anemia (low blood count) by helping you make red blood cells. Anemia causes fatigue and low energy levels. It occurs when there are not enough red blood cells to carry oxygen from the lungs to supply all the body's needs.


Macam ada seorang pakcik tu, nurse bagi subcutaneous EPO injection sebab dia ada renal failure. So, apa kena mengena EPO dengan renal failure?

Almost all the body's EPO is made in the kidneys. Kidney disease may damage the cells that make EPO, leading to anemia.

So kang kalau ada anemia, lain cerita lah pulak. Next injection untuk prostate cancer. Namanya ELIGARD. Nurse tu inject kat perut. 

ELIGARD is a prescription medication for the palliative treatment (management of symptoms) of advanced prostate cancer. ELIGARD is an LHRH (luteinizing hormone-releasing hormone) agonist. It is designed to reduce the amount of testosterone in the body by reducing the testicles' production of testosterone. It is not a cure for advanced prostate cancer.

Pakcik yang ada prostate cancer ni, banyak jugak lah past medical history dia. Termasuklah cardiovascular disease history. Tahun bila entah dia ada70-80%  ulcerative stenosis kat left circumflex coronary artery dia. So treatment dia ialah intracoronary nitroglycerin anticoagulation. Symptoms dia before being diagnosed dengan echo, dia ada unstable angina. Pastu pakcik ni jugak ada buat bypass surgery. 


Next, ada couple pakcik and makcik tua ni. Datang untuk sesi administrative information on how to use glucagon injection. Sebabnya pakcik tu ada Diabetes Mellitus Type 2. And selalu juga kena hypoglycemia and collapsed. So, nurse tu pun ajar makcik tu cara nak guna, siap suruh makcik tu praktis lagi, bagusnyaa! Nurse ni bukan sahaja suka ajar student, tapi juga suka mengajar pesakit, sangat cool~ Pastu tu cek telinga pakcik makcik tu sebab diorang mengadu macam ada masalah hearing. So, kita guna apa untuk cek telinga?





Otoscope namanya. Baru tahu lettew. HAHA. Tapi kedua dua telinga makcik pakcik tu okey je, takde la wax pun.


And then duk dengan GP jap, ada seorang patient ni dia minta renew prescription untuk depression dia. Tak ingat apa entah nama ubat tu. Aku tanya kat GP, sampai bila dia akan makan ubat tu? GP kata terpulang kat patient tu sebab patient tu pernah stop kejap, tapi dia maca unhappy and ambik balik ubat tu. And GP cakap jugak, kat Ireland ni sebenarnya banyak juga orang yang ada masalah depression. And banyak juga young people yang depress and end up bunuh diri.


And then sebelah petang pulak, pergi Pharmacy. Kedua-dua pharmacist tu baik sangat. Sangat friendly and they treat me like I am their sister. So, basically dia explain pasal-pasal ubat. And then arrangement of ubat-ubat kat rak tu. Banyak gila kot. And then lepas tu tolong isi ubat-ubat dalam bekas 7 hari seminggu tu. Dia jugak tunjuk administration stuffs macam file-file prescription bagai semua tu. Kalau ubat opiates dia letak dalam box berkunci kod bagai sebab itu controlled medication. Perhh.


Oh ya btw, pharmacist tu pesan, haha, nanti dah jadi doktor, tolong lah tulis prescription properly. Susah pharmacist nak baca kadang-kadang.


Okey now, let us make a quick revision mengenai beberapa jenis drug.


Zolpidem tartrate : sedative, also called a hypnotic. It affects chemicals in your brain that may become unbalanced and cause sleep problems (insomnia)

Lamotrigine : an anti-epileptic medication, also called an anticonvulsant

Olanzepine : antipsychotic medication that affects chemicals in the brain

Akineton : Biperiden is used to treat the stiffness, tremors, spasms, and poor muscle control of Parkinson's disease

Toviaz fumarate : reduces spasms of the bladder muscles

Duloxetine : selective serotonin and norepinephrine reuptake inhibitor antidepressant (SSNRI)

Senokot : Docusate is a stool softener. It makes bowel movements softer and easier to pass. Senna is a laxative. It stimulates muscle movement in the intestines. The combination of docusate and senna is used to treat occasional constipation

Lorazepam : group of drugs called benzodiazepines (ben-zoe-dye-AZE-eh-peens). It affects chemicals in the brain that may become unbalanced and cause anxiety

Tamsulosin : belongs to a group of drugs called alpha-adrenergic (AL-fa ad-ren-ER-jik) blockers. It relaxes the muscles in the prostate and bladder neck, making it easier to urinate. Tamsulosin is used to improve urination in men with benign prostatic hyperplasia (enlarged prostate).

Amiodarone : help keep the heart beating normally in people with life-threatening heart rhythm disorders of the ventricles (the lower chambers of the heart that allow blood to flow out of the heart)

Lipitor : belongs to a group of drugs called HMG CoA reductase inhibitors, or "statins." Lipitor reduces levels of "bad" cholesterol (low-density lipoprotein, or LDL) and triglycerides in the blood, while increasing levels of "good" cholesterol (high-density lipoprotein, or HDL)



Overall aku rasa, orang-orang kat Newmarket, kat Kanturk ni semuanya friendly habis. Even baru kenal pun, treat kita macam dah kenal lama je.
